Early Intervention Information
If there is a concern about a child’s development, the Early Intervention Program (EIP) can help by providing a free evaluation to find out if the child is eligible for services. Our program may be right for a family, if a child:
· Is not learning, playing, growing, walking or talking like other children their age
· Has a diagnosed condition that is likely to result in a delay in development
Anyone can make a EIP referral if the parent/caregiver would like to proceed with a referral by:
· Calling 311 and asking for “Early Intervention” or
· Completing the Early Intervention Program Referral Form (Fillable PDF) and faxing it to the Citywide Early Intervention Referral Unit at: 347-396-8801
Please note that the centralization of referral processing will not change any other established Early Intervention processes, including that:
Children will continue to be assigned to an Initial Service Coordination agency in the borough where they live. Follow-up questions about a child’s case should continue to be directed to the EI Regional Office in the borough where the child lives:
